2013 Dexter Athletic Physicals

2013 Dexter Athletic Physicals provided by Dexter Family Health Care

This is the easiest way to ensure your student athlete has the required information on file with the athletic department to allow participation in any sport!

Date: Friday, June 7, 2013

Time: 1-2pm for Middle School Athletes (students entering 7th, 8th, or 9th grade)
2-4pm for High School Athletes (students entering 10th, 11th, or 12th grade)

Location: Dexter High School – lines will form in the Commons Area (check in by main office)

Cost: $20 cash or check made out to Dexter Community Schools(all proceeds go towards the purchase of athletic training supplies for the 2013-2014 school year)

*A parent/guardian signature is required on the DHS Physical form.*

**Physical forms must be completely filled out with parent/student signatures**

***Physical forms can be found in the Athletics Office in Dexter High School or on the Dexter Community Schools Athletics web page under Athletic Information***
