Notes on Registration forms:

1. All forms must be printed and filled out.
2. You MUST bring a copy of birth certificate to the registrar. This is for prospective players that have not played for NP before, a copy of their birth certificate is needed for us to keep on file. We have to have proof of age for eligibility.
